Informed Consent for Telehealth Urgent Care Services
This consent is between you (the patient) and Sonali Health PLLC, a Texas-licensed telemedicine practice operated by Viola Sudheer, PA-C. By signing below, you acknowledge that you have read, understood, and agree to the terms of this document.
Nature of Telehealth Services
Your visit will be conducted via HIPAA-compliant video conferencing. Telehealth allows Viola Sudheer, PA-C to evaluate, diagnose, and treat certain acute medical conditions remotely. You understand that telehealth visits are different from in-person visits and have certain limitations.
Scope of Service
This urgent care telehealth service is designed for the evaluation and treatment of acute, short-term illnesses and minor injuries only, including but not limited to: UTIs, sore throat, ear infections, sinus infections, tooth pain (antibiotic bridge), muscle strains, colds and flu, conjunctivitis, mild skin infections, and GI upset.
This service does not include chronic disease management, specialist referrals, controlled substance prescriptions, psychiatric medication management, or emergency care.
This visit does not establish an ongoing primary care relationship.
Sonali Health does not provide FMLA documentation, disability paperwork, or legal medical records through this service.
Limitations of Telehealth
Your provider cannot perform a physical examination. Assessment is based on your reported symptoms, visual evaluation via video, and your medical history.
In some cases, your provider may determine that an in-person visit, lab test, or imaging study is necessary — and will advise you accordingly.
If your condition worsens or you experience any emergency symptoms, you must seek immediate in-person emergency care by calling 911 or going to the nearest Emergency Room.
Prescriptions
When clinically appropriate, your provider may send a prescription to your pharmacy of choice. Sonali Health does not prescribe controlled substances (Schedule II–V), including opioids or benzodiazepines, through this service. Prescriptions are based solely on the clinical information you provide — accuracy of your medical history and allergy information is your responsibility.
Emergency Situations
Telehealth is not appropriate for emergencies. If at any point during or after your visit you experience chest pain, shortness of breath, severe allergic reaction, signs of stroke, uncontrolled bleeding, or any other life-threatening condition — call 911 immediately.
Privacy & HIPAA
Your personal health information is protected under the Health Insurance Portability and Accountability Act (HIPAA). Sonali Health PLLC uses HIPAA-compliant technology for all video visits and patient communications. Your information will not be shared without your written consent except as required by law.
Financial Responsibility
The fee for this telehealth urgent care visit is $40 per consultation. Sonali Health does not bill insurance directly. You accept full financial responsibility for all consultation fees. Pharmacy and lab costs, if applicable, are separate and your responsibility.
Texas Residency Requirement
Sonali Health is licensed to provide telehealth services to patients physically located in the state of Texas at the time of the visit. By proceeding, you confirm you are currently in Texas.
Voluntary Consent
Your participation in telehealth services is entirely voluntary. You may end the visit at any time. Declining certain recommended treatments is your right, though your provider will discuss the potential risks of doing so.
